The Leadership Program has a unique structure and recommendations are used to assess whether the youth applicant will be successful in the program. Please ensure that you fill out the entire form - incomplete forms cannot be submitted.

This form must be accompanied by a 2012 Camp Application Form and a CIT Application Form. The youth applicant must submit these form before the application can be processed.
