This form must be completed by the CIT applicant, not a parent/guardian. Please complete this form in full to accompany your 2012 Camp Application Form. You will also be required to have someone complete and submit a CIT Recommendation Form on your behalf. Your CIT application will be processed once the 3 forms are received.
